As a SIRE Submitting Company you have the ability to search and review the incident database.
To search for an incident, click on Incidents in the menu bar at the top of most pages within SIRE. This will then take you to the search page:
Here you can enter vessel names or key words into the "Search Text" field as well as any relevant dates relating to an incident. Then click on the "Search" button below.
Once you have found an incident you would like to view, click on the button to the right of the incident listing to open it.
Before allowing you to view the incident a declaration will be displayed:
Click on the green "Agree" button to continue and view the report. This will then display the record:
You can then see the vessel details, at the top of the page. Under the "Incident Details" heading you will see the basic information relating to an incident including dates, locations and a brief description.
There are also four check boxes below this description:
Initial Notification
Incident Investigation
Root Cause
Corrective Action
As an investigation progresses, these stages will have check boxes checked when each one has been completed.
Most records also contain "Incident Documents" which provide further background information including investigation reports and further details of the incident circumstances. The documents can be downloaded from this area and viewed as required. Click on the icon next to a document to download and view it.
